News You Need to Know

  • Sport Physicals and Medical Release Forms- A current physical and medical release form MUST be on file in the 7th grade office before your students can tryout or participate in any HCCMS or HCHS sport.     Football Cheerleading- Tryouts will be held April 22nd-24th and is open to current 5th, 6th, and 7th graders. All students MUST have a current physical, medical release, and concussion form on file in the 7th grade office prior to tryouts. Please use the link below (in the sign-up section) to sign up.     HCCMS Softball Tryouts- Tryouts will be held on April 24th, 25th, 29th and 30th at HCHS from 4-6 p.m. Student must be an upcoming 6th, 7th, or 8th grader, have an updated GHSA physical, concussion awareness form & consent to treat form completed before the first day of tryouts. HCCMS students can get assistance form Mrs. Clark in the 7th grade office and Creekside students can get assistance from Mrs. Mace or Mrs. Rogers. Please use the QR code on the softball flyer or use the link under the sports tab on hccmsonline.com to sign up.

    HCHS Softball Tryouts- If you are a current or upcoming 8th grader and want to tryout for the high school softball team please pick up a flyer in the 7th grade office. To sign up use the QR code on the flyer or go to the Harris County High School website and click the sports tab, then click the softball tab. For more information please contact Coach Harrison at Harrison-s@harris.k12.ga.us.

     HCCMS Athletic Fees

    All athletes that make a HCCMS Athletic Team will be required to pay an athletic fee of $50.00 per sport. This fee covers payment for team t-shirt, banquet, awards, equipment maintenance, game day officials and security, etc.

    If a student/athlete is unable to pay the fee, an alternate plan will be utilized. Please speak with Athletic Director to work out a plan.


Requirements for Participation

    A.  Physical Examination
    A yearly physical examination is required for participation in any interscholastic athletics, including try-outs. This examination must be
    completed by a medical doctor on the form provided by Harris County Carver Middle School and submitted to the coach prior to participation.
    Physical forms will be kept on file in the athletic director’s office.
    B. Medical Consent Form  (Medical Release Form)
    Each athlete’s parent/guardian shall complete an Emergency Medical Authorization Form giving permission for treatment by a physician or hospital in the case of an emergency and when the parent/guardian is
    C.  Insurance
    The parent or guardian of each athlete must provide proof of medical insurance for each athlete before they may participate.
    D.  Athletic Eligibility
    Students must pass 70% of the subjects during the previous semester.
